Medication Authorizations

Maryland Health Regulations require forms  for campers who require any medications including over-the-counter medicines (aspirin, benadryl), asthma inhalers and Epi-Pen during camp hours.  All medication must be in original package.

The packet includes the following forms.

Immunization Records

Download this form only if the camper is not enrolled in a Maryland Public School:

If your child is not immunized, a contraindication statement from the child's physician or exemption by religious belief statement must be submitted before a child can be admitted to the camp. Download the form and have it completed by your physician.

Activity & Participation Release


Some camps have specific waivers due to activities such as white water rafting. These forms will be posted with the Camp Letters.

Use of Sunscreen During Camp

During Registration you will be asked to provide information on sunscreen use.

Parents have several options:

1. Allow staff to apply sunscreen provided by the County.

2. Allow staff to apply sunscreen provided by the parent.

3. Do not allow staff to apply sunscreen.

If the camper will be picked up by someone other than the parent who registered them, or the emergency contact listed on the registration form complete this form and email to CypressSwamp@calvertcountymd.gov 
  • Field Trip Pick Up Form (pdf) only required if someone other than authorized person is picking up camper from a field trip location (not the regular camp location).
